Under general supervision, provides, directs, supervises, and evaluates medical social services to clients for Madison County Health Care System in accordance with legal requirements, professional standards, and clinic policies. Assists patients and their families in navigating and accessing community services based on the patient-centered social needs. Advocates and provides social support and informal counseling to patients and families as needed. Must have at least a 4-year degree in Social Work (BSW / MSW). Previous medical social work experience in an ambulatory clinic, acute care hospital or skilled nursing facility preferred.
