Community Health Needs Assessment

Madison County Health Care System and Madison County Public Health are conducting a Community Health Needs Assessment (CHNA) to identify and analyze community health needs. The process provides a way for community partners to prioritize health needs and to plan and act upon unmet community health needs.

Information is gathered through several methods, including online surveys, community focus groups, interviews with stakeholders and feedback from the patients we serve. We know it is a busy time of year, but we hope you will take roughly 5 minutes to complete the survey to help us better identify the health need in our community.
